Couple Therapy Recording Agreement
This agreement outlines the terms and conditions under which therapy sessions between Naomi Light (hereafter referred to as "the Therapist") and the undersigned couple (hereafter referred to as "the Clients") will be recorded.
1. Purpose of Recording
The recordings will be used for the following purposes, subject to the Clients' explicit consent:
a) Therapeutic Purposes: Recording within the therapy session for immediate playback and review during the session to enhance the therapeutic process.
b) Professional Development: Recording and storing sessions for the Therapist's professional development, including use in supervision.
2. Consent
The Clients hereby provide their consent to the recording of therapy sessions as follows:
a) Consent for Therapeutic Purposes:
I/We agree to the use of recording within the therapy session for immediate playback and review during the session for therapeutic purposes.
Both partners to consent to Part A:
b) Consent for Professional Development:
I/We agree to the recording and storing of videos of the session by the Therapist for the purposes of professional development, including use in supervision.
Both partners to consent to Part B:
3. What We Do with the Information We Gather
The Therapist requires the information outlined in section 2 and the session recordings for the purpose of offering therapeutic services in accordance with their privacy and ethical policies. All information is held in strict confidence.
4. Data Protection and GDPR Compliance
a) The Therapist is committed to protecting the privacy and confidentiality of the Clients' personal data in accordance with the UK General Data Protection Regulation (GDPR).
b) Recordings will be processed lawfully, fairly, and in a transparent manner.
c) Data Minimisation: Only recordings necessary for the specified purposes will be made.
d) Storage Limitation:
i. Recordings used for therapeutic purposes (as per 2a) will be disposed of at the end of the therapy session in the presence of the Clients.
ii. Recordings used for professional development (as per 2b) will be stored securely and retained only for as long as necessary for that purpose.
e) Data Security: The Therapist has implemented appropriate technical and organisational measures to ensure the security of the recordings and protect them against unauthorised access, disclosure, alteration, or destruction. These measures include encryption and two-factor authentication.
f) Confidentiality: The recordings will be kept confidential and will only be accessed by the Therapist and, where applicable, the Therapist's supervisor, for professional development purposes.
g) Clients' Rights: The Clients have the right to:
i. Withdraw their consent at any time. Withdrawal of consent will not affect the lawfulness of processing based on consent before its withdrawal.
ii. Request access to their recordings (subject to limitations where disclosure may negatively impact on the confidentiality and/or privacy of another individual).
iii. Request rectification of any inaccurate recordings.
iv. Request erasure of their recordings (subject to legal and professional obligations).
v. Lodge a complaint with the Information Commissioner's Office (ICO) if they believe that the Therapist has not complied with data protection law.
5. Use of Recordings for Professional Development
a) Recordings used for professional development will be used solely for the purpose of enhancing the Therapist's clinical skills and will be viewed only by the Therapist and their supervisor.
b) The Clients' anonymity will be maintained when recordings are used for professional development. Any identifying information will be anonymised where possible.
6. Controlling Your Personal Information
a) The Therapist will never sell, distribute or lease your personal information to third parties unless required by law to do so.
b) Clients may request details of personal information which the Therapist holds about them. Requests should be sent to hello@naomilight.com.
c) Clients may also request that the Therapist erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
d) If any information is believed to be incorrect or incomplete, please contact the Therapist as soon as possible. The Therapist will promptly correct any information found to be incorrect.
7. Changes to this Agreement
The Therapist may change this agreement from time to time. Clients will be informed of any significant changes and asked to provide renewed consent where necessary.
